Know a teen or young adult student (ages 14–22) who is starting to plan for life after high school? Encircle Families and Diverse Ability Incorporated are here to help. Our Family Transition Navigator provides personalized information, training, referrals, mentoring, and systems navigation support so families can move confidently from school-based services to adult systems.
The program’s goal is simple and powerful: to help families, including youth and young adults, gain the knowledge and skills they need for the journey ahead. We walk beside you as you explore postsecondary education, employment, and independent living. Along the way, we clarify legal rights, service eligibility, and how to coordinate supports across key Arizona agencies, including:
- Vocational Rehabilitation (VR) for career exploration and job training
- Division of Developmental Disabilities (DDD) for ongoing supports
- AHCCCS for health care coverage and related services
