What It Is
Family Transition Navigation helps families and transition-aged youth who have disabilities (ages 14–22) prepare for life after high school. We know the journey from school-based services to adult systems can feel overwhelming. Our Family Transition Navigator provides personalized support so families can understand their options, make informed decisions, and build skills for the future.
We focus on three key areas:
- Employment – exploring job opportunities, training, and supports.
- Education – understanding paths to college, vocational programs, or other postsecondary learning.
- Independent Living – planning for daily life skills, housing, transportation, and community participation.
Our program is grounded in empowering both families and young people to take an active role in shaping their futures.
How It Works
When you connect with our Family Transition Navigator, you’ll receive:
- Information & Training – Learn about transition planning, legal rights, service eligibility, and timelines.
- Referrals – Get connected with trusted agencies and community resources.
- Mentoring & Guidance – Receive support from someone who understands the process and is committed to your family’s success.
- Systems Navigation – We’ll help you coordinate services from agencies like Vocational Rehabilitation (VR), the Division of Developmental Disabilities (DDD), and AHCCCS.
Support is tailored to your family’s needs, whether you’re just beginning to think about transition or working through the final steps.
